If you have a passion to make a difference, we are looking for people like you to join our exciting new West Kent health partnership.  This includes Sevenoaks District, Tunbridge Wells and Tonbridge & Malling Borough Councils working alongside Kent County Council.  

We are looking for two 'One You Kent' Advisers to deliver  integrated healthy lifestyle work throughout the Sevenoaks District area for an initial two year period.

Your key role will be to carry out 1:1 health and wellbeing assessments, follow ups and to record outcomes.  You will deliver a range of targeted outreach healthy living interventions on weight management, physical activity, healthy eating and brief intervention on smoking cessation, alcohol intake (following appropriate training), and attend community events to promote the service.  You will work alongside local health professionals and key voluntary sector partners in connecting services around the customer.

You will need to be outcome focused with proven working knowledge and experience, ideally within a health related service.  You will also have the drive, creativity and communication skills to change people's lives.
